It was created by producers Mary-Ellis Bunim and Jonathan Murray. The first cast consisted of seven people who were paid $2,600 to appear on the series. The cast was filmed living in a SoHo loft from February 16 to May 18, 1992, The series premiered May 21 that year. This was the first of three seasons to be filmed in New York City. In 2001 the show made a repeat visit with The Real World: Back to New York, and in 2008, set its twenty-first season, The Real World: Brooklyn, in the borough of Brooklyn.

The season, which documented 22 weeks of the cast's interactions, began with two cast members being flown to Nashville and then spending ten days driving cross country to Venice Beach in a Winnebago, picking up a third cast member in Owensboro, Kentucky along the way.

This conflict provided considered what Entertainment Weekly calls emotional high points for the season, and are credited with making The Real World a hit, for which it was ranked #7 on Time magazine's list of 32 Epic Moments in Reality-TV History. The season is also notable for featuring the first-ever same-sex commitment ceremony on TV, between Zamora and his partner, Sean Sasser.

To give the show a new music twist, the seven strangers work as "modulators" at alterna-rock radio station, 107.7 The End. What begins as a series of promotional duties eventually leads to producing and hosting a live radio show. The gang has to deal with major issues ranging from the perils of long-distance relationships to suicide. Later in the season, the cast members travel to Nepal in support of their various radio station duties. And who can forget Stephen's goodbye slap for Irene?

It is notable for the cast's concerns over castmate Ruthie Alcaide's excessive drinking, resulting in her leaving the house to enter an alcohol treatment program, which is cited as one of the series' memorable moments by Time magazine.
